Advance Care Planning Facilitator Hospital Palliative Care Team

An exciting opportunity has risen for an enthusiastic, highly experienced and motivated nurse or an allied health care professional  to join the specialist palliative care team at Kingston hospital. The team works in an advisory role, supporting patients with advanced disease requiring specialist palliative care and their family.

The advance care planning facilitator will work with clinical teams supporting recognition of potential for dying and opportunities for advance care planning. The post holder will support and facilitate timely discharge to patients preferred place of care/death and lead on developments for rapid discharge of dying patients. Education and role modeling for the multidisciplinary teams is core in enabling them to support a positive experience for patients and their loved ones.

You will be a registered general nurse or allied health care professional with appropriate qualifications and experience of being involved with caring for patients with advanced incurable disease. This includes excellent interpersonal skills, an up to date knowledge base, and the ability to work autonomously.
